Do Butalbital-Containing Products Have a Role in the Management of Migraine?

Abstract and Introduction

Abstract

Study Objective: To evaluate the role of butalbital-containing products in the management of migraine.
Methods: Qualitative systematic search using MEDLINE (January 1966-November 2001), review of the United States Headache Consortium's evidence-based guidelines for migraine treatment, and review of other pertinent literature.
Results: Over 28 million people suffer with migraine, yet this illness is less than optimally diagnosed and managed. Between 14% and 36% of diagnosed migraineurs are prescribed butalbital-containing products, often as initial therapy. However, the only identified controlled trial of these drugs for migraine treatment showed that butalbital-containing products were inferior to butorphanol. The consortium's guidelines specifically discourage administration of butalbital-containing products for migraine. In addition, other published literature highlights the frequent adverse consequences of butalbital-containing products for migraineurs, such as poor migraine control, disability, drug-induced headaches, and withdrawal symptoms.
Conclusion: Although butalbital-containing products commonly are prescribed for migraine, no evidence in the literature demonstrates their benefit over other agents or placebo. Drugs with proven migraine efficacy, as listed in the consortium's evidence-based guidelines, should be prescribed instead.

Butalbital is in a class of drugs known as barbiturates and is used to treat tension headaches. There are currently no reported nutrient or herb interactions involving butalbital. See barbiturates for interactions common to this class of drugs, though they have not yet been investigated for butalbital.

I take this type of med for my migs and it works for me. I dont know if it does for others though.

I started taking Fiorinal at about 16 years old. At that time migraines were infrequent.....one every couple of months or so.

They worked for me for several years, but around the age of 20 or so...they were no longer effective

For me...if I can manage to get to sleep (knock myself out) and get a pain killer into my body, I wake (usually) in no pain.

I think thats why the Fiorinal worked for me for a time. It calmed me down and made me tired so that i cuold get to sleep. Well..maybe thats why it worked. Why it stopped working..I'm not sure

beth_taylor, how come the article mentioned butorphanol? Stadol, as it is more commonly known is entirely different than butalbital and I believe unarguably stronger. Is it comparing the two for efficacy in migraine patients? The article just appeared a bit biased.

I used Esgic Plus as a teen-ager for years with great results. To this day, I find Fiorinal or Fiorinal with Codeine to help significantly with mild to moderate migraines. HH, maybe Fiorinal with Codeine would knock you out and be helpful again.

I use Fiorcet and it works okay for me. You've got to eat prunes afterwards!
Now that I've discovered triptans I use fiorcets less often.

Wow, that's odd...I take Fiorinal and it really works for my migraines...I should say, it helps me sleep and works for the headaches that are smaller type migraines.

I have tried both Fiorinol and Fioricet, and Fioricet with codeine, and the first two did absolutely nothing whatsoever, I have put them into the OTC category- better known as cereal.

The Fioricet with codeine just made me very loopy and sick to my stomach for hours on end, but that was the codeine part. I am just very sensitive in a negative way to any pill with any amount of codeine or the codones, so I can't take any of those. Oh well, I am still looking for that miracle pill!
